The 2025-2026 4-H Enrollment Form can be printed, completed, and mailed to our office at 4800A New Hartford Rd, Owensboro, KY 42303. You can also complete it electronically and e-mail it to one of the 4-H Agents: Stacey Potts and Sharayha Clingenpeel.
